| Electronic Submission |
If your company or organization allows you to send and receive attachments with your email please fill in the form completely and use the submit button to send electronically. Upon receipt BGI will assign it an RMA number and email it back. |
| Fax Submission | If your company or organization does not allow you to send or receive attachements with your email please fill in the form completely, use the print button to print the form and then fax to 781-647-1607. Upon receipt BGI will assign it an RMA number and fax it back. |
